A lot of games have free Live codes (usually 48 hrs) in them. A good way to get them is buy used games for cheap from Gamefly. The cases are brand new and all inserts (including manual and codes) are intact and unused.

At Gamefly you rent games so the codes are probably already used, unless Gamefly replaces the codes every time the game gets returned.

No, the codes are in the original case which only gets shipped if you buy the game. Every code I've gotten from a game purchased from Gamefly has worked.
